Enrollment Agreement:
The Agreement entered into between ________________________ hereafter called applicant and the Reflexology Science Institute, hereafter called Institute on _______________, 20____. Whereas, the applicant has made application as a student in the Certification in Reflexology program offered by the Institute for the ____________________ Term which begins on ____________________, 20_____.
Whereas, when the Institute has accepted the applicant as one of its students, now this agreement:
1. The APPLICANT agrees to attend regularly, to abide by the Institute's regulations, to complete the prescribed course of study in which he/she is enrolled, and agrees to pay the registration fee and tuition fee for said course as set forth in this agreement.
2. It is understood that in fixing the payment for instruction at the amount indicated, liability of the nature hereafter mentioned and the cost of protection against same have been excluded and therefore, I hereby release the Institute (for myself, my heirs, executors, administrators, representatives, and assigns) of and fro, all liability whatsoever that may arise from any sickness or injury, or loss or damage to personal property which I may suffer or incur on or about, or by reason of being on or about said school and other premises of the Institute, or the premises of other persons to which I may be sent during the period I received instruction hereunder.
3. Tuition payments must be made in advance of the term. The initial payment to be made with this enrollment agreement shall not be less than the required Registration Fee. A student is then regarded as enrolled.
4. The Institute has the right to dismiss the student for misconduct, unsatisfactory progress, unexplained absences or a delinquent tuition account.
5. I understand that the tuition charges are for the right to attend classes for which I am enrolled, and in no way contingent upon satisfactory progress, personal satisfaction, or receipt of financial aid.
6. The Institute has an informal and formal complaint procedure which is listed in the school catalog. A student may direct any complaint or problem to the Executive Director of the Board of Career Colleges and Schools (614-466-2752), 35 E. Gay Street, Columbus, Ohio 43215, whether or not the problem or complaint has been resolved to his satisfaction.
7. The training takes approximately six months to complete. The program includes 112 class room hours.
8. I have received the Institute's catalog and understand it is part of this enrollment agreement.
Registration Fee:
The registration fee is $50.
Tuition Fee:
For students enrolled in 2007, the Tuition Fee for the Certification in Reflexology program is $1850.00| tuition fee of | $1850.00 |

Additional Fees or Expenses:
The fee for the required textbook and foot charts for Certification in Reflexology is included in the tuition fee of $1850. Students are required to receive 1 foot Reflexology session from the instructor. Students receive a 20% discount from the normal fee. As of June 2006, the normal fee is $75, making the student fee $60.
Refund Policy:
The following policy regarding refunds will be adhered to by the Institute:
1. If the enrollment application is not approved, all monies you paid the Institute will be refunded.
2. The Institute requests oral or written notice of withdrawal. If this is not done, the date of withdrawal shall be the last day of recorded attendance.
3. The applicant student may cancel the enrollment at any time prior to attendance. All but $35 of the registration fee paid by the student shall be refunded. Once the student has started school, the registration fee is not refundable.
4. If a student withdraws during a term, partial refunds of tuition will be made on a declining basis, depending on the length of time a student has remained enrolled. The following schedule of refunds apply to tuition charges only.

5. Tuition refund is based upon the payment of the full amount
of tuition.
6. The following charges are not refundable once the student begins school: Registration Fee, cost of purchasing books and supplies.

This enrollment agreement may be canceled by mailing a written notice to the Institute postmarked no later than the 5th business day after this agreement is signed. The Institute will promptly refund the registration fee and all tuition fees collected.
